Diabetes Directions
At Saint Francis Hospital and Health Centers, the Diabetes Directions team is committed to providing patients with the knowledge and skills necessary to manage diabetes. As part of the multidisciplinary program, patients meet with a Certified Diabetes Nurse Educator and Certified Diabetes Registered Dietician who assist them in attaining the knowledge and skills necessary to manage diabetes on a daily basis. The Diabetes Directions team collaborates with Primary Care Providers to develop a plan of care that is individualized to meet each patient’s needs.
